Your care team includes a hospice physician, nurses, nursing assistants, volunteers, and a spiritual & grief counselor who are familiar with your community.
Agrace is a nonprofit, community-supported health care organization that provides hospice care, palliative care, dementia support and grief support to residents throughout southern Wisconsin, as well as in select areas of southeastern Minnesota and northeastern Iowa. Most hospice care is provided in patients’ homes—wherever they call home, which could be a relative’s home or an assisted living or skilled nursing community.

Yes. Our hospice care services come to you. We help many patients stay in the place they feel most comfortable, whether that’s their home or inside a care community.
Hospice care is specialized care for people nearing the end of life. It focuses on comfort, dignity and quality of life rather than curative treatment.

Palliative care helps manage symptoms of serious illness and can be provided at any stage of illness, alongside curative treatment.

Hospice care and palliative care are usually covered by Medicare, Medicaid or private medical insurance.
Agrace also offers free or discounted services through our Care for All program for those who qualify financially. No one is turned away due to inability to pay.
No. You do not need a doctor’s referral to start the conversation with Agrace. Anyone can refer a person for hospice care.
