Agrace HospiceCare is accepting new patients for end-of-life care, including people who have tested positive for COVID-19.
PalliaHealth (palliative care) consultations can be scheduled either in person or via telehealth (video chat).
Grief support: You can make appointments for one-to-one, in-person grief support (with physical distancing) at the Agrace Grief Support Center in Fitchburg, or at our Janesville, Baraboo and Platteville offices. If you prefer not to meet in person, grief support visits can also happen over the phone or by video chat using a smartphone, tablet or computer.
Grief support visits are open to anyone in the Agrace service area, even if the person who died was not a hospice patient. Please call (608) 327-7110 to make an appointment.
Visiting Agrace: Visitors are screened for COVID-19 symptoms before they may enter Agrace’s hospice inpatient units. Please bring your own face covering or mask to wear. If you do not have a mask, or you are visiting a patient in contact isolation, Agrace will provide a mask. This mask guidance is based on recommendations from the CDC.
All patients may have three visitors per day, and more if they are at the end of life. If you have any questions before visiting Agrace, please call (800) 553-4289.
