Learn about the supportive care and hospice services available to support your patients/residents and their families.
Agrace is here to help you learn about supportive or end-of-life care with a variety of resources. Our staff can assist you as you talk to people in your care and their families about our services, including end-of-life matters.
Agrace Hospice Care is an open access hospice: Agrace patients may receive concurrent services such as chemotherapy, radiation therapy, blood transfusions or dialysis while they’re on hospice if the patient’s goal in receiving treatment is to relieve pain or other symptoms of their terminal illness, rather than to try to cure the illness. Agrace considers these requests individually, so please ask us about your specific case.
Advanced Cardiac Care program: In collaboration with the American Heart Association and the National Partnership for Hospice Innovation, Agrace clinicians receive specialized training to manage the symptoms of advanced heart disease. This extra layer of care improves comfort and quality of life for both our hospice and supportive care patients. This program is certified in Palliative/Hospice Heart Failure by the American Heart Association. Learn more here.
Making a referral: If you think a person in your care could benefit from Agrace’s Hospice Care or Supportive Care services, call (800) 930-2770 at any time to ask questions or make a referral. We also have a simple online referral form:
We will do our best to visit with your patient (or family/guardian, if appropriate) in the next day or two following the referral. Agrace staff will meet with them to learn what’s happening with them and recommend services that best meet their needs, whether that is Hospice Care or Supportive Care, a palliative care program.
