Agrace is a nonprofit, 501(c)(3) health care agency that serves the entire community. We rely on donations like yours to bring care and support to people who are aging, living with serious illnesses, dying and grieving. All donations and fundraising for Agrace are coordinated by the Agrace Foundation.

The Agrace Foundation accepts donations via the following methods:

• Donating securely with a credit card here.
• Writing a check payable to “Agrace Foundation” and mailing it or dropping it off to Agrace Foundation, 5395 E. Cheryl Parkway, Madison, WI 53711

How Gifts are Used

Your support of the Agrace Foundation ensures that Agrace can provide exceptional care to every person who needs it, regardless of their circumstances. Insurance does not fund several vital services and programs—like those below—that are at the heart of Agrace’s mission:

• Care for All: Funding for patients or clients to receive services from Agrace if they lack insurance and have no other way to pay
• Community Grief Support: Emotional support available to everyone in the community who is grieving a death
• Clinical Advancement: Scholarships, specialized training, and community partnerships that help Agrace’s staff grow, excel and deliver the best possible care to our patients and clients
• Dementia Support Programs: Expert dementia care and education for Agrace patients, clients and the broader community
• Patient Wish & Essential Needs Fund: Fulfilling patients’ end-of-life wishes and meeting other needs essential to their daily comfort and quality of life

Promises to Keep: When your investment in Agrace totals $10,000 or more (cumulatively), you are invited to join our Promises to Keep Circle. Your name or your loved one’s name can be added to our donor-recognition walls at Agrace in Madison or Janesville.

Legacy Circle: Donors who share in a commitment to the future of Agrace by making estate gifts are invited to join our Legacy Circle. You may have your name or your loved one’s name listed on our donor-recognition wall.
