Honoring the Memory of Your Loved One

We often hear from people who want to make a donation to Agrace in memory or honor of someone special. Agrace welcomes this support, because we rely on donations to help fulfill our nonprofit mission of compassionate care for patients and their families. This page describes several ways you can make a gift.

What happens after I make a gift?

When you donate in memory of an Agrace patient, the Agrace Foundation will notify their family member(s) we have on file.  Your name and address will be listed, but not the donation amount.

How are families notified of memorial gifts?

  • Agrace will mail a letter to the person(s) listed as the patient’s primary contact(s), showing the names and addresses of those who gave memorial gifts.
  • Letters are mailed once a month (usually mid-month) and list any gifts made in the previous month.
  • Agrace will continue to send monthly letters for as long as memorial gifts continue to be made in their loved one’s memory.
 

Including Agrace in an Obituary

Agrace is grateful to receive memorial donations that are requested in your loved one’s obituary. Here is an example of some language you may choose to use:

Memorial donations may be made in [name]’s name to Agrace, 5395 E. Cheryl Parkway, Madison, WI 53711 or at Agrace.org/Donate.

 

Forwarding Memorial Donations

If you received a check in memory of a loved one that you would like to donate to Agrace, you may endorse it to the Agrace Foundation. Then mail it or bring it to Agrace, 5395 E. Cheryl Parkway, Madison, WI 53711.

How to endorse a check:

If you wish to endorse a check over to Agrace, sign your name in the endorsement area on the back of the check and write “Pay to the order of Agrace Foundation.”

 

How to Honor an Agrace Staff Member or Volunteer

If a staff member or volunteer made your experience with Agrace special, we encourage you to let us know and make an optional donation in their honor. Fill out this brief form so we can recognize their exceptional care.

Share Your Story

Every family’s time with Agrace is unique, and your story has the power to inspire and bring comfort to others facing similar experiences. Sharing your loved one’s Agrace story can be a meaningful way to celebrate their life while helping others understand the impact of Agrace’s compassionate care.

Purchasing Memorial Bricks and Butterflies

You can create a lasting tribute that also supports Agrace by purchasing a memorial brick or butterfly. These tributes add beauty to Agrace’s gardens, and you may visit them anytime.
