Agrace brings hospice care to you—so you can stay in the familiar comfort of home.
Agrace Hospice Care comes to you, wherever you live. This could be your own home or apartment, a family member’s home or the Agrace Residence in Madison. You can also have hospice care while you live in an assisted living, nursing or other community residence. Agrace partners with more than 150 long-term care facilities in our service area to provide this additional layer of care to their residents.
Your nurse, social worker, nursing assistant and other hospice caregivers visit you, so you can stay in a place that’s familiar and comfortable—instead of traveling to a clinic. We teach your family and other caregivers how to help you between our visits, and we explain when to call Agrace for extra support.
We bring health supplies, and if needed, we arrange for medical equipment to be delivered. We make sure you know what services, medications and treatments are needed and are covered by your hospice benefits.
You and your family can call Agrace any time of day or night if you have a concern or a symptom that’s causing you worry.
If you have pain, breathing difficulties or other health problems that cannot be managed at home, your nurse may recommend that you come to one of Agrace’s two inpatient units (IPUs) for care—usually for just a few days—until you are well enough to return home.
