Agrace, the largest Wisconsin-based nonprofit hospice, has expanded to serve the entire southern half of Wisconsin with the addition of Ozaukee, Washington, Racine and Kenosha counties to its Milwaukee territory.
For nearly 50 years, Agrace Hospice Care has offered compassionate end-of-life care to people of all ages in southern Wisconsin. Southeastern Wisconsin residents can now choose Agrace for hospice care that comes to them where they live—in private homes, long-term care facilities or any other place they call home.
Hospice care is for people with a prognosis of six months or less. Agrace also offers palliative care through virtual medical consultations. Agrace Palliative Care is for adults who want help managing the symptoms and other quality-of-life challenges of a life-altering illness. This service is available at any point in the illness, regardless of prognosis.
People who choose Agrace locally are served through its regional office in Wauwatosa, by a care team that includes local hospice physician Andre Cernasov. Dr. Cernasov oversees care plans for hospice patients and their families throughout southeastern Wisconsin.
Applications for local volunteers are now being accepted.
Volunteers are needed to help make companionship visits to local hospice patients, which may involve reading aloud, listening to music together, playing cards, assisting with meal delivery or simply being a caring presence. No health care experience is required. Volunteer schedules are flexible, and free training is provided. For more information, or to fill out an application, visit Agrace.org/Volunteer.
Anyone can refer a patient to Agrace Hospice Care. A doctor’s referral is not required to contact us and ask about care. Agrace admits patients to hospice seven days a week, including holidays. For questions or to request an informational visit, call (414) 245-2135 or visit Agrace.org/Refer.
