Visiting Agrace:

Visitors must self-screen for COVID-19 symptoms and sign in at the reception desk before they can visit Agrace’s hospice inpatient units and other areas of our buildings.
Masks are recommended while at Agrace and will be available at the reception desk. Visitors are not required to wear masks in patient rooms unless it’s the patient’s request or signage on the patient room door indicates otherwise.

Living at the Memory Care Suites

Agrace’s Hospice Memory Care Suites are a homelike, secure area on our campus in Madison where hospice patients can receive support for their memory care needs.

Very few hospices are able to offer this option, but thanks to generous community support, Agrace can!

What is it like to live there?

Rooms are large and private, with your own private handicap-accessible bathroom.
You have views of the gardens and nature.
Furnishings include an adjustable bed, sleeper sofa, recliner, small dining table and guest chairs.
Family and friends can visit anytime and stay overnight in your room.*
Your room has its own temperature controls and WiFi.
Meals are created to be comforting and satisfying. Agrace uses many locally sourced foods to support area farms and food producers.
Your hospice services are equivalent to those Agrace provides in a patient’s home or other private residence: a registered nurse and social worker oversee your care, and most daily care is provided by nurses and certified nursing assistants (CNAs).

*Visitors, and access to some regularly available amenities, may be limited for safety during the COVID-19 outbreak.

*For safety, residents of the Hospice Memory Care Suites must be accompanied by a family member or Agrace staff.

What does it cost?

Your hospice services are paid for by Medicare or health insurance, as they would be if you were receiving care while living at home.
Separately, you pay a daily residential services fee (also called “room and board”). This covers the cost of your room, housekeeping, utilities, meals and access to all of the common areas and amenities at Agrace. This fee is $435/day (as of January 1, 2024).
There is also a refundable security deposit of $4,000, which is due when you move into the Memory Care Suites (this deposit is not required for short-term stays).

Getting Started

You must be enrolled in hospice care with Agrace to live in the Agrace Hospice Memory Care Suites. If you have not already contacted Agrace about hospice care, please call now (800) 930-2770.

If you are already enrolled (or plan to enroll) with Agrace for hospice care:

Read our application packet (download a printable copy here). It includes a cover letter, an application form and Agrace’s Residence and Memory Care Suites Policies.
Print and fill out the application packet, then review it with your Agrace care team (or your enrollment representative if you do not yet have an assigned care team).
Sign your application form and the last page of the policies to acknowledge that you understand and agree to all policies. Give all six pages of the signed packet to our staff, who will return it to Agrace.

When a room is available and you are ready to move, Agrace will review your medical records and health status to ensure that we can safely meet your care needs in our Hospice Memory Care Suites.
After you are medically approved, Agrace will contact your care team or enrollment representative to arrange a date to move.
Upon moving in, you will pay a $4,000 security deposit (this does not apply to short-term stays), as well as the rest of the current month’s residential services fee. Please review the Residence/Memory Care Suites Policies for detailed payment information.
