What is the Patient & Family Partnership Council?

Was someone close to you served by Agrace? Would you like to share your experience to help Agrace continue to improve?

Please consider joining Agrace’s Patient & Family Partnership Council, it’s a small group of volunteer advisors who saw Agrace’s care and support up close when their family member or friend was a patient with Agrace. Council members give constructive feedback that helps guide and improve Agrace’s services.

 

What does the Council do?

The Council brings together family advisors and Agrace staff each month to talk about subjects that are important to hospice patients and their families. Agrace leaders listen, to better understand what families need from Agrace while their loved one is receiving end-of-life care. Then we consider how we can apply the Council’s insights into our care, services, publications and future projects.

You don’t have to have any special background or training; we simply want your opinion. Council members have given their ideas about our admission process, preventing patient falls, grief and spiritual support, our Patient & Family Care Handbook, our instructions for giving comfort medications—and much more.

Suggest a Topic

Do you have topics or ideas you believe the Council should discuss? Please use our online form to suggest a topic. On this form, direct your message to “patient and family relations.”

 

Apply to Join

You may be a good fit for the Council if you:

  • experienced Agrace’s support while a family member or friend was a patient with Agrace,
  • feel comfortable discussing your experience and ideas in a small group setting,
  • are interested in helping Agrace continue to improve our services,
  • want to have a voice in how end-of-life care is designed and delivered, and
  • can meet with the rest of the Council at Agrace once a month.
