1095 Tax Information for Current and Former Agrace Employees

Active Employees can access their W-2 and 1095 tax forms through their Agrace Single Sign-On (SSO) login at https://paycom.agrace.org

Instructions for former Employees:

1. Visit Employee Self-Service
2. Click “Forgot Username” to reset your username and password
