Visiting Agrace (effective May 4, 2023):

Visitors must self-screen for COVID-19 symptoms and sign in at the reception desk before they can visit Agrace’s hospice inpatient units and other areas of our buildings.
Masks are recommended while at Agrace and will be available at the reception desk. Visitors are not required to wear masks in patient rooms unless it’s the patient’s request or signage on the patient room door indicates otherwise.

Dean-o Trevino Golf Outing

Dean Frederickson, beloved husband, father, grandpa, and friend, passed away from cancer in September 2017 surrounded by his family at the Agrace Hospice facility in Fitchburg, Wisconsin. Not only was Dean blessed with such wonderful care in his final month, but the support and care given to the family will never be forgotten.

Dean lived a wonderful life to the age of 87. And in those years, he had two passions– Family and Golf! To say he was an avid golfer would be an understatement. It was what he missed most in his final years.

To honor his love of both family & golf, his family will host the 1st annual Dean Frederickson- or Deano Trevino-Golf Outing! Saturday September 22 at 8:00 AM at the Door Creek golf course in Cottage Grove. For more information go to the Agrace Facebook page.
