Agrace is open for hospice and palliative care during the COVID-19 outbreak. Learn more here.
Hospice care and palliative care consultations improve your comfort and quality of life. These services come to you, throughout the Agrace service area.
Agrace HospiceCare is accepting new patients for end-of-life care, including people who have tested positive for COVID-19.
PalliaHealth (palliative care) consultations can be scheduled either in person or via telehealth (video chat).
To request either of these services, please call us now at (800) 930-2770.
Visiting Agrace: For everyone’s safety, you will be screened for COVID-19 symptoms before you may visit Agrace’s hospice inpatient units. Please bring your own face covering or mask to wear. If you do not have a mask, or you are visiting a patient in contact isolation, Agrace will provide a mask. This mask guidance is based on recommendations from the CDC.
All patients may have three visitors per day, and more if they are at the end of life. If you have any questions before visiting Agrace, please call (800) 553-4289.
