Agrace Selected as Preferred Hospice Provider to Crossing Rivers Health

Agrace, Wisconsin’s largest nonprofit hospice, is pleased to announce it is working with Crossing Rivers Health to take over the health system’s hospice program. The two organizations have also entered into a Preferred Partner Agreement to support the local community’s ongoing needs for high quality end of life care. This agreement comes three months after Agrace’s preferred partnership with Oakwood Village Communities and supports Agrace’s rapid growth across the state of Wisconsin.

Crossing Rivers Health, based in Prairie du Chien, has decided to close its hospice service line to focus on their core healthcare services. Current Wisconsin Crossing Rivers Health Hospice patients will have the option to transition to Agrace’s care by the end of September.

Agrace Receives 2023 Top Workplaces USA Award

Today Agrace announced it has earned the 2023 Top Workplaces USA award, issued by Energage. Agrace was named #79 of the top 100 companies in the country among those with an employee base of 500 to 999 workers. The Madison-based health care nonprofit employs more than 850 people who work to fulfill its mission of providing hospice care, supportive care, non-medical senior care, adult day services and grief support.

Winners of the Top Workplaces USA list are chosen based solely on employee feedback gathered through an employee engagement survey issued by Energage. In the past 16 years, the Top Workplaces program has surveyed more than 20 million employees and recognized the top organizations across 60 markets for regional Top Workplaces awards. Energage results are calculated by comparing the survey’s research-based statements, including 15 Culture Drivers that are proven to predict high performance, against industry benchmarks.

Agrace Thrift Store to Celebrate Achieving Dementia-friendly Status at April 18 Ceremony

Thanks to special staff training, the Agrace Thrift Store on Madison’s West Side is being recognized as “dementia-friendly” by the Aging & Disability Resource Center (ADRC) of Dane County.

On Thursday, April 18, at 10:30 a.m., Agrace Thrift Store–West staff, volunteers and supporters will host a ceremony celebrating the store’s new “dementia-friendly” status. The community is invited to dress in purple for dementia awareness and visit the store at 118 Junction Road, Madison, for a ribbon-cutting presentation.

Hospice Patient Sees Packer Game in Wish Come True

Francis Thompson, an 86-year-old Prairie du Sac resident and life-long Green Bay Packers fan, had a wish come true October 15 when he was able to attend the Packers’ Monday Night Football game at Lambeau Field. Francis’ wish was made possible by his Baraboo-based nurse, CNA and social worker from Agrace HospiceCare.

“After a lifetime of watching the Packers play, this was Francis’ first time seeing them play in person,” said Tina Hohn, Thompson’s nurse case manager with Agrace. “Francis talks about it every time I see him. He says, ‘I still can’t believe I made it to the game!’”

Editorial: Step up to help Agrace meet matching grant

Agrace Hospice & Palliative Care, the area nonprofit that regularly is praised for the quality of its end of life services to thousands of local people, needs your help.

Back in August, a group of donors in honor of Agrace’s 40th anniversary combined to pledge $400,000 to the nonprofit’s campaign to raise money for its “Care for All” endowment.

Just one catch: The donations required Agrace to raise a matching $400,000 by Oct. 14.

Unfortunately, the campaign is in danger of falling short of the match. While Agrace has been able to raise $250,000, it needs some good donations in the coming week to reach the match.

Employees put on Race for Agrace to help patients

FITCHBURG (WKOW) — Employees at Agrace Hospice are helping their patients pay for care.

The Race for Agrace is a one-mile fun walk and timed 5K to support the nonprofit Care for All Foundation. Care for All provides free or discounted services to patients who can’t afford end of life or palliative care.

The run was organized by Agrace employees who wanted to help their patients.

“We hope through the race to raise about $33-thousand. Also really exciting is we have a match campaign right now going for our Care for All Foundation, so that those dollars should be able to be matched to help us achieve the final $1 million that we hope to raise for Care for All,” said Chris Hammer, a nurse with Agrace.

This was the second annual Race for Agrace.

Agrace is a non-profit, community-supported agency with offices in Madison, Janesville, Baraboo and Rockford.

Dean-o Trevino Golf Outing

Dean Frederickson, beloved husband, father, grandpa, and friend, passed away from cancer in September 2017 surrounded by his family at the Agrace Hospice facility in Fitchburg, Wisconsin. Not only was Dean blessed with such wonderful care in his final month, but the support and care given to the family will never be forgotten.

Agrace Adds Members to Foundation Board

Agrace Hospice & Palliative Care has appointed Karen Christianson, Judith Martinez, Terry Murawski and Bill Schendt to its Foundation board. Agrace is Wisconsin’s largest nonprofit community hospice and palliative care agency, providing end-of-life care and related services to people in southern Wisconsin and northern Illinois. The Agrace Foundation Board guides fundraising, helping to meet the current and future needs of the organization.
